What is Country Care Connect?

Country Care Connect is a new GP‑led, multidisciplinary mental health initiative designed to improve access to integrated, multidisciplinary care for people with severe and complex mental health needs across the Darling Downs and West Moreton region.

The program is establishing four service delivery locations - Chinchilla, Goondiwindi, Gatton and Esk.

The program is funded by Darling Downs West Moreton Primary Health Network and supported by Health Workforce Queensland.

A Multidisciplinary Care Model

These services will deliver culturally safe, trauma‑informed, place‑based mental health services close to where people live. Each service will be situated within a general practice, and will be supported by a telehealth psychiatry service.

The service will introduce a psychologist, and social worker or mental health nurse to each site, who will work closely with GPs, allied health and the broader mental health system to deliver excellent services that are underpinned by strong system coordination.
