The Practice Nurse position is responsible for the provision of culturally appropriate efficient and responsive comprehensive primary health care to our Aboriginal and Torres Strait Islander people within a primary health care setting. The service adopts a holistic approach which focuses on the concept of continuity of care and comprehensive care for Aboriginal and or Torres Strait Islander people.

Salary Package - Competitive base salary + 9.5% super + salary sacrifice + 5 weeks annual leave
Key Duties Include:
- Deliver clinical services within a comprehensive primary health care service environment with an interdisciplinary team focus, which is:
- of high quality

- Provide day to day health service activities to the designated communities within the context of Primary Health Care in a professional, confidential and culturally safe manner
- Provide effective first point of medical engagement including, triage assessments, taking of medical history/detail, assignment of patient, completion of preliminary procedures
- Provide routine screening services to patients including blood pressures, blood sugar levels, temperatures, urine testing (routine and pregnancy tests), weight, length and head circumference (in children under three)
- Provide treatment and care to patient under the direction of the doctor including immunization of children under five years old against childhood diseases as by the doctor
- Conduct Adult Health checks, Immunization of adults as requested by the doctor
- Treatment of wounds and other clinical procedures as directed by the doctor
- Liaise and coordinate treatment, care and follow up through consultation with other sections in the service, hospitals and agencies
- Attend to home visits with doctor if required
- Provide health education and promotion on a one to one basis to all the services clients
- Provide counselling and support for patients and referrals to appropriate sectors through consultation with doctor and patient
- Patients are informed of respiratory etiquette, hand hygiene, and precautionary techniques to prevent the transmission of communicable diseases
- Immunisations recommended by Australian Immunisation Handbook are provided based on individual patient's immunisation status
- Patients are recalled for any significant results and are advised of any follow up tests and results
- Provide evidence of Immunological status for Vaccine Preventable disease and if required be agreeable for the administration of vaccine
- Maintain Treatment /Consulting Room ensure fully stocked, checked and tidied after each client as per Clinic Procedure
- Use equipment as per procedures, report any functional problems, maintain sterilization procedures
- Provide all clinical care within scope of practice
- Under limited supervision store, prepare and administer medication in accordance with unit policy, protocol and relevant legislation
- Instruct client/care givers on how to take medication, the correct dosage, storage and security
- Instruct client/care givers on how to obtain new medication supplies when required.

Key Requirements Include:
- Registration with AHPRA & licensed to practice as a Registered Nurse.
- Relevant clinical experience and recency of practice with commitment to evidence-based outcomes
- A demonstrated understanding of the needs of Aboriginal and or Torres Strait Islander Community Controlled Health Services (ACCHS) and interest in comprehensive primary health care nursing within the Galangoor Duwalami Model of Care.
- Responsive to the needs of Aboriginal and Torres Strait Islander people and has received or willing to attend training in cultural reflective practice.
- Excellent communication skills with patients, their families and with the interdisciplinary team
- Commitment to Professional Development of self and others to encourage lifelong learning.
- Ability to plan, prioritise and time manage.
- Utilises available resources effectively and efficiently for the delivery of evidence-based nursing health care.
- Current Cardiopulmonary Resuscitation (annual), and First Aid Training (every 3 years).
- Current Class C driver licence.
- Current Blue Card.
- Satisfactory Police Check.
- Evidence of immunisation against vaccine preventable diseases
Desirable:- Membership of appropriate professional body
- Experience working as a Practice Nurse in a primary health care setting
- Experience working in an Aboriginal and Torres Strait Islander Health Service and/or working with Aboriginal and Torres Strait Islander people.
- Holds an Immunisation Nurse Practitioner certificate
- Experience in Phlebotomy and/or IV cannulation (or willingness to work towards developing this skill)
- Experience in Implanon insertion / removal (or willingness to work towards developing this skill)
- Experience in cervical screening testing (or willingness to work towards developing this skill)
- Experience in Chronic Disease Management e.g., care coordination, chronic disease self-management, preparation of GPMPs (Medicare Item 732)
