Aboriginal Health Worker/Practitioner – ACCHO – Full-Time - Charleville, Queensland

Position Description:
The purpose of this position is to provide a clinical service within a culturally sensitive environment to support a smooth and efficiently functioning clinic, in addition to providing an exceptional standard of care to their clients. To undertake liaison, health promotion, screening and care planning, to improve chronic disease prevention, detection and management for Aboriginal and Torres Strait Islander people. To promote, communicate and facilitate increased engagement, health literacy and health education as part of Primary Health Model of Care within a culturally sensitive environment.
Position Requirements and Key Responsibilities:
Advocacy:
Engage, encourage and assist Aboriginal/Torres Strait Islander community members in a professional, confidential and culturally safe manner:
To provide advocacy and representation on behalf of clients
To build sustainable working relationships with health care providers
To ensure you engage allied health services that provide cultural security and safety to their clients
To ensure informed consent is obtained for Health professionals prior to treatment
Clinical Service Delivery:
Perform clinical duties within required level of clinical competency, according to best available evidence.
Duties include: triage, wound management, chronic disease management, ECGs, spirometry and collection of pathology samples
Assist doctors with clinical procedures and maintain clinical documentation
Explain procedures to patients, providing them with support and reassurance
Ensure whole of community approach to service provision and client care plans in partnership with allied health services
Patient Services:
Assist with triage, data management, diagnostic services, networking with other providers, planning and management of patient care
Telephoning patients for follow-up as directed by medical staff
Improvement of Client Health Outcomes:
Conduct preventative/screening procedures including the Indigenous Health check to identify client risk factors for chronic disease
Assist with client education and community health promotion activities
Deliver health promotion programs to promote health and well-being
Action patient recall and GP outreach programs, GP management plans, and team care arrangements
Patient Visits:
Provide home/hospital visits as required for monitoring support, including antenatal and post-natal care and health assessments
Equipment and Supplies:
Ensure sterilisation and maintenance of clinical equipment
Maintain stocks of clinical supplies, including correct storage (such as refrigeration), removal and disposal of out-of-date stock and ordering supplies
Compliance:
Maintain awareness of current and new legislation to ensure clinics are complying with all statutory and regulatory obligations including infection control sterilisation, hazardous materials and safe handling/disposal of medical waste, records management, WHS, and accreditation
Ensure relevant personnel are kept informed and changes are made to systems and procedures as required
Infection Prevention and Control:
Maintain systems and processes for hand hygiene, environmental cleaning, and spills management
Educate all members of the clinic team on an annual basis regarding their responsibilities in infection prevention and control, or whenever changes in legislation, or clinic procedures occurs
Manage sterilisation process, ensuring sterility and integrity in the process
Ensure the clinic's waste management complies with local, state and federal regulations.
