Practice Information:
This practice works with the local Aboriginal and Torres Strait Islander community and relevant key stakeholders to increase access to culturally safe, coordinated, holistic comprehensive primary health care services. They focus on the management and prevention of chronic disease in the of the Fraser Coast Region.

Position Description:
As the Social Worker, you will provide efficient and responsive social, emotional health and wellbeing primary health care services to the Aboriginal and Torres Strait Islander community, focusing on addressing mental health, trauma and drug and alcohol use, ensuring integration within the model of care, in a professional and culturally safe manner.
Key Responsibilities:
Delivery of efficient and responsive social, emotional, health and wellbeing services to clients
Provide counselling and psychological services specifically regarding issues of grief, loss and trauma resulting from past government policies and where this has led to excessive risk-taking behaviours, e.g., alcohol and other drugs, and these impacts upon families
Enabling an integrated service within a model of care which ensures a flexible responsive approach to services
Be main point of contact and access for clients with psychosocial needs, directing from various internal and external pathways and referrals.
Ensuring evidenced-based social worker assessment, care / intervention, evaluation and follow up is holistic encompassing social, emotional, mental, physical, health and wellbeing of individuals and the whole community – (e.g., individual, families, family groups, country, culture, spirituality and language)
Strengthening and maintaining culture in all service delivery ensuring provision of culturally safe spaces
Empowering patients and families with self-determination to make their own decisions and take control of their health, (e.g., health education and promotion)
Regularly provide service promotion and outreach services to enable access for clients, including facilitation of group support sessions and healing workshops
Build and maintain strong relationships, and fostering of partnerships at an advanced level with key community stakeholders
Demonstrate clinical leadership within the multidisciplinary team to meet the changing needs of individuals and the community, e.g., Sharing of knowledge, liaison and coordination of treatment, care and follow up.
Manage portfolio consistent with skill level and level of practice
Practice safety by demonstrating professionalism and accountability in comprehensive health promotion practices
Understand and address the unique needs of Elders
Monitor, evaluate and report outcomes of clinical services in line with both funding outcome measures, and the practice’s primary health care KPIs and targets
Qualifications and Experience:
Bachelor and/or Master of Social Work with eligibility for membership with the Australian Association of Social Workers (AASW). If overseas trained – approval letter of proof of verification with AASW must be submitted
A new-graduate Social Worker may be considered for this role who has an ability to work autonomously with appropriate levels of supervision and support within a multidisciplinary team
Experienced in not only counselling but care coordination, report writing, familiar with Child Safety legislation etc
Experience working with Aboriginal and Torres Strait Islander people, as well as knowledge of current social, emotional, health and wellbeing issues affecting these communities
Experience in mental health issues, domestic and family violence and alcohol and substance use
Exemplary interpersonal communication skills, including verbal and written skills.

Market base salary
10% super

4 weeks annual leave
Monday to Friday, 8.30am – 5.00pm

Maryborough a heritage city, situated on the Fraser Coast, offering the best of country to coast lifestyle. The city has a population of around 28,000 and draws patients from surrounding rural areas. Hervey Bay is a 30 minute drive on good roads, and close to access for Fraser Island. Maryborough is about 250 km north of Brisbane.
