Employment Type: Permanent –Part-Time (2 - 4 days per week).

About the Role:
Duties include:
Patient triage
Patient assessment
Therapeutic care and treatment
Clinical data management
Communicating with other services
Integration of service delivery
Planning and management of care
Providing information and feedback between the services, patients and GP
Patient advocacy
Infection control
Cold chain monitoring
Records management
Occupational health and safety
Accreditation process
Health information
Education
Self-care/self-management education
Immunisation
Recalls
Patient education
Systems management
Acute and chronic disease management
Assistance with procedures/Excisions
Position Requirements:
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
Minimum of 2 years' experience as a Registered Nurse, preferably in a GP setting preferred
Strong clinical skills and knowledge of evidence-based nursing practices
Excellent communication and interpersonal skills to work effectively with patients, families and the healthcare team
Demonstrated ability to work autonomously and as part of a collaborative team
Commitment to continuing professional development and providing high-quality patient-centred care

About Toowoomba:
Perched on the edge of the Great Dividing Range, Toowoomba is considered the capital of the 'Darling Downs' Region. Located 127kms or a 90-minute drive west of Brisbane, Toowoomba has a population of about 105,000 and is situated approximately 700 metres above sea level.
